The application form will ask the following questions (you might want to complete your application in a document before inputting it into the form, as the form cannot be saved and returned to):
- Your full name
- Your pronouns
- Your email address

- Please provide a statement (no more than 100 words) describing your current work.
- Please provide a short summary (no more than 100 words) about how you would use your time at Hospitalfield by describing a specific project or area of your work that you would like to develop.
- Please upload your CV* (max 1 A4 page, saved as a PDF) - this document will give us a sense of what stage you are in your practice.
*In your CV, where applicable please include:

- Your name, address and any education history - you do not have to have studied art or studied at all to apply.
- Any previous or forthcoming exhibitions (solo or group), performances, awards or grants, residencies, or any other moments in your artistic journey that you are most proud of.
- Please also include links to your website or your artist social media – it’s fine if you don't have either.
- If you have little or no exhibition history or experience as an artist, please don’t worry – this opportunity is for people who would like to take their work to the next level, and connect with a powerful, supportive network!
- Please upload up to 3 examples of your work, including titles, media, durations or dimensions and year. As part of the 3 works, you can include up to 1 hyperlink to AV work uploaded to Vimeo, Youtube or Soundcloud.
- Feel free to let us know about any access requirements, mobility considerations, or health-related factors you'd like us to be aware of.

Unfortunately, we are unable to provide artists with feedback on their application due to the volume of applications. We’re actively looking to facilitate this in the future.
